How to use Questionnaires?

Questionnaires can be inserted into to contents of any Medical Record and there filled up. For example, you can insert questionnaire into a consultation by opening a consultation and pressing the Insert Questionnaire link. Once inserted you can fill up the form the by clicking on the links. You can continue typing text into the content..

Can I define my own Questionnaires?
