Integration in Patient Chart
You can find the integration in Patient Chart -> History as above
By default the system will display the history templates from above (Medical, Surgical, Social, Family history).
You can switch from the displayed template to another by pressing it's associated link

Commands
1. Edit Values - will open the Form Editor. Next Simply enter the values in the form, and press Save and Close
2. Edit Template - will open the Form Template Designer and you can design the layout of the current template
| Edit Template command is enabled only for Administrator users |
3. Layout -> Manager Layout - allows each user to choose which form templates to show and in what order
4. Layout -> Add Form - allows the adding of another template in the list of templates
