Patient Forms are customizable data entry forms. Form templates can be defined from Form Templates using the Form Template Designer

Standalone Forms

These can be found in Records->Activities->Forms, or under Patient Chart->Forms

Add new Patient Forms

1.  Press the New Form button to add new form

Select the template for the form and fill out the form's fields with the Form Editor 

Edit existing Patient Forms

1. Double-click on any record in the list in order to edit it

Delete an existing Patient form

1. Select a Entry item and click the Delete button on the toolbar.
2. Click Yes in the confirmation box.
